About the Role:

We are seeking a highly motivated and experienced Holistic Care Coordinator to join our Homecare team in the South Metro region. Based at our Port Kennedy Homecare office, this role will involve travel between all Bethanie sites and services.

The successful candidate will work with the Homecare team to manage all aspects of client onboarding for new clients as well as ongoing case management for a cohort of clients prior to transfer to colleagues in the case management team.

About You:

  • You have a passion for delivering person-centred care and making a positive impact on the lives of others.
  • You have current knowledge of the Aged Care Standards and Homecare Case Management.
  • You have experience developing and managing client budgets and are comfortable working within a team environment.
  • You possess excellent organisational skills and ability to manage multiple priorities.
  • You have strong verbal and written communication skills and are proficient in using client database systems.
  • You hold a valid driver's licence and have access to a reliable vehicle.
  • You are willing to obtain required vaccinations – COVID 19 (minimum of 3 doses) and annual flu vaccination.
Key Responsibilities:
  • Oversee and coordinate the onboarding process of new Home Care Package clients for the assigned region.
  • Develop ongoing relationships with local networks and external agencies to strengthen access to services and supports for daily living that are important for the client's health.
  • Assist clients in identifying ongoing needs and goals in order to support them to remain in their own home.
  • Monitor and manage client budgets in line with approved package level and individual client requirements.
  • Maintain reports, information and support as requested by the Regional Homecare Manager to a high standard.
What We Offer:
  • A competitive salary of $65,000 - $80,000 per annum, depending on experience.
