The Transition Care Program is a goal-oriented, strength-based, reablement program that focuses on progress towards goals and outcomes. As a key member of our multidisciplinary team, you will provide support and assistance to enable clients to lead a continued safe and independent life within their own home.

In this role, you will be responsible for providing personal care, including showering, dressing, and continence care. You will also assist clients with the administration of correct medication and adhere to Client Care Plans. Additionally, you will assist clients with domestic duties, meal preparation, and other household tasks to ensure safe and hygienic living conditions.

We are looking for someone who has at least 12 months experience as a full Community Support Worker to the aged and competency in the administration of medications. This role requires flexibility to work shifts that may require coverage Monday to Sunday with a mix of morning and evening shifts.

Key Requirements:

  • Current Australian driver's licence and reliable car - essential
  • Past experience working in Aged care as a Community Support Worker is essential and this must include the delivery of personal care
  • Medication competency
  • Certificate III Aged Care - desirable
  • Dementia Care experience - desirable
  • Current First Aid - desirable

Estimated Salary: $65,000 - $75,000 per annum (dependent on experience)
