We are seeking an experienced Client Care Specialist to join our Homecare team in the South Metro region, covering areas from south of the river to Mandurah and surrounding suburbs. This role involves travel between Bethanie sites and services to ensure active case management and meet the needs of Homecare Service clients.

This permanent full-time position offers a salary of $85,000 - $95,000 per annum, with flexible working arrangements available. The successful candidate will work closely with the Homecare team to manage client onboarding and ongoing case management for a cohort of clients prior to transfer to colleagues in the case management team.

About the Role:

  • Oversee and coordinate the onboarding process of new Home Care Package clients for the assigned region.
  • Develop relationships with local networks and external agencies to strengthen access to services and supports for daily living that are important for client health.
  • Assist clients in identifying ongoing needs and goals to support them in remaining at home.
  • Monitor and manage client budgets in line with approved package levels and individual client requirements.
  • Be an effective member of the multi-disciplinary team to ensure high standards of holistic care are delivered.
  • Maintain reports, information, and support as requested by the Regional Homecare Manager to a high quality and in accordance with policy and procedures.

About You:

  • Willingsness to uphold Bethanie's Mission, Vision, and Values.
  • Experience in homecare case management or a clinical background with more than 3 years in a similar organisation.
  • Knowledge and experience within a person-centred care context within the Aged Care industry.
  • Knowledge of Case Management principles, Home Care Package Guidelines, and Funding body.
  • Ability to work within a team and autonomously.
  • Excellent organisational skills and ability to manage multiple priorities.
  • Excellent verbal and written communication skills and strong computer skills with experience using client database systems.
  • Valid driver's licence and access to a reliable vehicle.
  • Current National Police Clearance or willingness to apply.
  • Willingness to obtain required vaccinations (COVID-19 and annual flu vaccination).
