We are seeking an experienced healthcare professional to provide case management and assessment within a multidisciplinary team structure.
About the RoleThis is a temporary leave relief position with the possibility of extension. The role provides time limited, specific, evidence based, community focused person centred care, with consideration to the specific needs of the target group of consumers referred to the Older People's Community Mental Health Team.
Key Responsibilities- Deliver high quality case management and assessment services.
- Work collaboratively within a multidisciplinary team structure.
- Registered Nurse qualification.
- Experience in case management and assessment.
The successful candidate will have the opportunity to work with a dedicated team and contribute to delivering high quality patient care.
