Transform Lives as a Lifestyle Coordinator
This role is an exciting opportunity to make a positive impact on the lives of others by delivering a personalized, meaningful lifestyle experience for each resident.
You will gather life history profiles and work closely with residents, families, and the multidisciplinary care team to develop individualized lifestyle plans that reflect each resident's cultural, spiritual, emotional, and personal interests.
Working alongside the therapy and OT teams, you will help design and coordinate tailored group and individual programs, promote participation, and ensure residents are informed about upcoming activities. You'll support and guide Therapy and Leisure Partners in adapting programs to resident needs, help plan special events and celebrations, and build community connections to enrich the lifestyle offering.
This is a dynamic role that blends planning, creativity, and collaboration, with the goal of enhancing resident wellbeing through personalized and culturally inclusive engagement.
Key Responsibilities:
- Develop and implement individualized lifestyle plans that reflect each resident's unique preferences and needs.
- Design and coordinate tailored group and individual programs to promote participation and engagement.
- Work collaboratively with the therapy and OT teams to adapt programs to resident needs.
- Plan special events and celebrations to enhance the lifestyle offering.
- Build community connections to enrich the lifestyle experience.
Requirements:
We are seeking a dynamic and compassionate individual with excellent communication skills and the ability to manage a busy workload. A formal qualification in Health, Aged Care, Allied Health Assistance, or Community Services is preferred, but not essential.
The ideal candidate will have experience working in aged care and possess a solid understanding of WH&S legislation and practice. They will be able to lead a small team and respond flexibly to the needs of the residents.
