Relationship-Based Care Specialist

The Role:

This is a key position in delivering high-quality care that values every individual as a whole person.

Responsibilities:
  • Provide emotional and spiritual support to residents, families, and staff
  • Be the point of contact for internal and external support services
  • Coach staff to deliver relationship-based care
  • Develop care plans and conduct assessments using HOPE tools
  • Liaise with managers, case managers, and attend meetings as required
  • Train staff on palliation and wellbeing
About You:
  • Experience working with people living with dementia
  • A strong Christian faith and church involvement
  • Relevant qualifications and experience in pastoral care
  • Excellent communication skills, including listening, compassion, and empathy
