Caring for You Nursing Agency are looking for a dynamic driven person to champion our Home and Community Care department, a Registered Nurse with Home Care experience who is ready for a change, challenge and to make your mark on an industry you are incredibly passionate about.

**About us**

Caring for You is a family-owned and run business, we have been in operation since 2004, and while our expertise traditionally lies within the Aged and Acute care sector we have always provided exclusive high-level home and community care to a range of private and brokered home care recipients.

**About the Role**

As an experienced Client Adviser/Case Manager, you will work collaboratively with our current home care coordinators, clients, carers, and representatives to develop, deliver and support key stakeholders.
- Review and collect intake information, engage new clients, establish home care agreements, care plans, and liaise with stakeholders and suppliers to provide nothing but 5-star care.
- Implement new processes, documentation and reporting in line with our ISO 2009 standards.
- Oversee a team of Home Care coordinators and be accountable for their performance, growth, and HR needs.
- Interact with care providers, stakeholders, and partnered service providers representing the best interests of clients and the values of Caring for You.
- Conduct quarterly client visits to review care plans, goals, and service provisions to ensure quality standards and service guarantees are met.
- Ensure service provisions are conducted ethically, fairly, and within the statutory legal and contractual requirements.
- You will champion services provisions tailored to the needs of individual clients, including the development of care plans in consultation with key stakeholders.
- Provide professional and competent day-to-day delivery of services.

**About You**
- Registered Division 1 Nurse** **with a minimum of 2 years of relevant industry experience.
- Knowledge of the Home Care Packages Program, Aged Care Quality Standards, SCHCADS and Commonwealth Home Support Programmes.
- A current driver's license and willingness to travel between clients, sites, and services.
- Strong administrative and computer skills 40 WPM minimum.
- Amazing references who will support your team focused work ethic and demonstrated results.

**The Package**

Reporting to the General Manager of Allocations you will be supported in growing the Home Care Department, you will be valued as a key stakeholder in the department as well as a member of the Caring for You Leadership Team.
