Job Description

Clinical Aged Care Assessment Officers are required to work within the Single Aged Care Assessment System (SAS) team. The role involves conducting client assessments, reviews and support plans to assist clients in achieving their goals and living independently.

The officers will participate in clinical triage and case conferencing, identifying, monitoring and managing risks, and leading appropriate escalation.
