Job Title: Community Physiotherapist

Job Summary:
HealthcareLink is seeking a skilled Physiotherapist to join our team in the Melbourne community. As a Physiotherapist, you will be working within a multidisciplinary team to provide high-quality Physiotherapy assessments and interventions for consumers in the Aged Care and Disability Sectors.

Key Responsibilities:

  • Provide Physiotherapy assessments and interventions for consumers in a professional, confidential, and culturally safe manner.
  • Complete consumer-focused Physiotherapy assessments to manage a range of musculoskeletal, neurological, and cardio-respiratory disorders.
  • Ensure integrated service delivery and follow-up, coordinating and communicating as required.
  • Incorporate a holistic approach to healthcare that includes health promotion activities for consumers.
  • Ensure accurate and timely recording of information in the management system and maintain records in accordance with Aged Care Quality Standards and company policies.
  • Ensure privacy and confidentiality are maintained in relation to consumer information.
  • Provide manual handling and education training for carers and family members.
  • Consult with site staff, doctors, consumers, family, and other care professionals to develop consumer care plans, goals, and interventions.
  • Regularly review consumer outcomes and make necessary changes to align with consumer goals.
  • Document each consumer treatment and general care in clinical notes as per company guidelines.
  • Attend and participate in meetings, including multidisciplinary, staff, and management meetings.

  • Excellent communication and organizational skills.
  • Current Working With Children Check.
  • Valid Australian working rights.
  • Full Registration with AHPRA.
  • Vaccination in line with government mandates.
  • Have your own car and valid driver's license.
