Physiotherapist - Community Care Specialist

Salary: $75,000 - $100,000 + super + travel allowance
Job Description:
We are seeking a dedicated and passionate Physiotherapist to join our multidisciplinary team in delivering exceptional quality care within the Aged Care and Disability sectors.
Key Responsibilities:
- Provide high-quality Physiotherapy assessments and interventions for consumers within the context of Aged Care, maintaining professional, confidential, and culturally safe standards.
- Conduct consumer-focused Physiotherapy assessments to manage musculoskeletal, neurological, and cardio-respiratory disorders.
- Ensure integrated service delivery and follow-up, coordinating and communicating as required.
- Implement a holistic approach to healthcare, incorporating health promotion activities tailored to consumers’ priorities and requirements.
- Accurately and timely record consumer information in the Management system, adhering to Aged Care Quality Standards and company policies.
- Ensure privacy and confidentiality in relation to consumer’s information.
- Provide manual handling and education training for carers and family members.
- Collaborate with site staff, doctors, consumers, family, and other care professionals to develop consumer care plans, goals, and interventions.
- Regularly review consumer outcomes and make necessary adjustments to align with consumer goals.

- Excellent communication and organisational skills.
- Current Working With Children Check.
- Valid Australian working rights.
- Full Registration with AHPRA.
- Vaccination in line with government mandates.
