Job Role Overview:

We are seeking a professional to join our team in rolling out grant funding under the CHSP Growth Funding. This is an exciting opportunity for someone with experience in grant funding and service provision.

The successful candidate will have expertise working with the Commonwealth Home Support Programme (CHSP) grant funding and its service provision, as well as registration and service delivery under My Aged Care.

The role involves investigating and applying for additional grant opportunities within the disability and aged care sectors, coordinating home maintenance services, and maintaining financial records including performance reports, tax invoices, and compliance under the grant.

Main Responsibilities:

  • Researching and submitting applications for additional grants in the disability and aged care sectors
  • Coordinating the delivery of home maintenance services
  • Maintaining accurate and timely financial records including performance reports, tax invoices, and compliance under the grant
  • Collecting and reporting wellness and reablement data
  • Collaborating with multidisciplinary teams, families, and other stakeholders to ensure holistic care and support
  • Maintaining accurate and up-to-date documentation and reporting any issues as needed
  • Adhering to all safety protocols and guidelines to guarantee a safe working environment
