Job Description

We are seeking a skilled and compassionate Senior Community Care Coordinator to join our team. In this role, you will be responsible for coordinating care services and ensuring the highest quality of life for our residents.

Key Responsibilities:

  • Care Service Coordination: Develop and implement individualized care plans in consultation with residents, families, and healthcare professionals.
  • Service Delivery: Coordinate and manage the delivery of care services, including medication administration, personal care, and domestic assistance.
  • Communication: Maintain effective communication with residents, families, staff, and external stakeholders to ensure seamless care coordination.
  • Quality Assurance: Monitor and evaluate care services to ensure compliance with regulatory requirements and organizational standards.

Required Skills and Qualifications:

  • Degree in Social Work, Nursing, or related field.
  • Minimum 2 years' experience in aged care or community service coordination.
  • Strong communication and interpersonal skills.
  • Ability to work independently and as part of a team.
  • Basic computer skills and knowledge of care management software.
