Care Coordinator

About the Role:
  • We are seeking an experienced individual to match clients with caregivers based on their specific requirements.
  • Schedule and coordinate in-home care services to ensure seamless support and comfort for our clients.
  • Become the vital link between families, caregivers, and healthcare professionals, facilitating open communication and effective care delivery.
  • Maintain and update client care plans to reflect changing needs and circumstances.
  • Ensure compliance with industry standards and best practices to guarantee exceptional care and outcomes.
  • Deliver outstanding customer service and support to our valued clients, addressing their concerns and empowering them to make informed decisions.
What We're Looking For:
  • A minimum of 2 years' experience in aged care/disability care and knowledge of NDIS standards and Aged Care Standards is highly desirable.
  • Strong organizational and communication skills are essential for success in this role.
  • Ability to multitask and work effectively in a fast-paced environment is crucial.
  • A caring and compassionate attitude towards our clients and their families is paramount.
  • Proficiency with scheduling software and basic computer skills are necessary for efficient task management.

Certificate III/IV in Individual Support (Ageing and Disability) or equivalent qualification is required for this position.
