
Care Coordinator Specialist

We are seeking a compassionate and dedicated professional to join our team in Newstead, TAS. This full-time role is a unique opportunity to contribute meaningfully to the lives of our residents by coordinating and delivering social, emotional, and recreational support.
The ideal candidate will have at least 2 years of experience, a person-centred approach, strong communication skills, and a passion for enhancing the quality of life for older people.

Key Responsibilities- Plan, coordinate, and implement lifestyle and social care programs tailored to individual resident needs
- Foster a supportive and engaging environment through meaningful activities and therapeutic services
- Build positive relationships with residents, families, and the wider community
- Maintain accurate documentation and contribute to care planning
- Work collaboratively with the broader care team to promote holistic wellbeing
- Minimum 2 years' experience in a relevant role
- Certificate IV in Leisure and Lifestyle or Diploma in Leisure and Health (or willingness to obtain)
- First Aid Certificate
- Proficient in computer systems and information management
- Excellent communication and interpersonal skills
- Ability to work independently and as part of a multidisciplinary team
- Understanding of aged care quality standards and person-centred care principles
- National Police Check and appropriate working rights in Australia
- Experience in delivering complementary therapies and recreational services
- Demonstrated experience in dementia care
- Experience working in residential aged care settings
- Knowledge of and connections with local community groups and support agencies
