
Care Liaison Specialist

This role is part of our commitment to supporting the local Aboriginal and Torres Strait Islander community. The successful candidate will work closely with local Elders, older Aboriginal and Torres Strait Islander people and their families to raise awareness about their aged care entitlements.
About the RoleThe primary responsibility of this position is to ensure that support for clients is organised, sequenced, and supported to enable effective access to appropriate aged care services both internal and external to the organisation. This includes working under the supervision of a Co Ordinator- Intake, Assessment and Engagement to determine the level of local demand for aged care services, identify existing aged care services, any service gaps in the area and continuing advocacy work with clients as required.
Key Responsibilities- Establish and maintain strong relationships with the local Aboriginal and Torres Strait Islander Community.
- Develop and implement strategies to increase awareness of aged care entitlements among the target population.
- Work collaboratively with other teams to provide comprehensive support to clients.
- Identify and address service gaps in the community.
- Provide ongoing advocacy and support to clients as needed.
To be successful in this role, you will need:
- Excellent communication and interpersonal skills.
- Experience working with older people and understanding of the internal services available to clients.
- Knowledge of or willingness to learn about the aged care landscape.
- Ability to work unsupervised and as part of a team.
- High level of personal resilience and flexibility.
