Aged Care Liaison Specialist

About this Role
This position has primary responsibility for connecting with local Elders, older Aboriginal and Torres Strait Islander people and their families to raise awareness of aged care entitlements.
The successful candidate will work under supervision to ensure support for clients is organised, sequenced, and supported to enable effective access to appropriate aged care services both internal and external to the organisation.
This role requires strong relationships with the local Aboriginal and Torres Strait Islander Community, excellent communication and interpersonal skills, experience working with older people, understanding of available internal services, knowledge of or willingness to learn about the aged care landscape, and ability to enter data and reporting.
Key Responsibilities:
- Determine the level of local demand for aged care services
- Identify existing aged care services and any service gaps in the area
- Continue advocacy work with clients as required
What You Need to Succeed
The ideal candidate will have a high level of personal resilience and flexibility, with the ability to deal with challenging situations, and be willing to undergo background checks, obtain necessary clearances, hold current certifications, and be fully vaccinated against COVID-19.
