Lead Care Coordinator

We are seeking an experienced and compassionate Lead Care Coordinator to join our team. As a key member of our care management team, you will play a vital role in delivering high-quality care services to elderly Australians.

Job Description

As a Lead Care Coordinator, you will be responsible for coordinating the delivery of care services to clients across various settings. Your primary focus will be on ensuring that clients receive safe and effective care that meets their individual needs and preferences.

You will work closely with a multidisciplinary team of healthcare professionals, including nurses, therapists, and support staff, to develop and implement comprehensive care plans. Your expertise in aged care and experience working with Home Care Packages and Commonwealth Home Support Program (CHSP) will be invaluable in this role.

Key responsibilities will include:

  • Developing and implementing individualized care plans that meet clients' unique needs and preferences;
  • Collaborating with the care management team to ensure seamless care coordination;
  • Providing education and support to clients and their families on care-related topics;
  • Monitoring and evaluating the effectiveness of care services delivered;
  • Participating in quality improvement initiatives to enhance care outcomes;
Required Skills and Qualifications

To be successful in this role, you will need:

  • A Certificate III in Individual Support or equivalent qualification;
  • At least 2 years of experience in aged care, preferably in a care coordination or management role;
  • Knowledge of Home Care Packages, CHSP, and Aged Care Quality Standards;
  • Current First Aid Certification and CPR skills;
  • The right to work in Australia;
  • A current driver's license;
  • National Police Clearance;
