Job Title:

Comprehensive Assessor

About the Role:

We are seeking a skilled and dedicated Comprehensive Assessor to join our multi-disciplinary team.

Key Responsibilities:

  • Conduct comprehensive and independent assessments of clients, focusing on aged individuals and some younger adults when appropriate, within the framework of the Aged Care Assessment Program (ACAP)
  • Develop care plans that promote functional restoration and maximise independence
  • Review support plans and facilitate referrals or provide service coordination for older people to engage community or hospital-based services
  • Collaborate across health and community sectors to ensure sustainable, well-managed, and efficient service delivery

Required Skills and Qualifications:

  • Ability to adapt and operate within a changing service environment
  • Excellent communication skills, including verbal and written
  • Strong time management and organisational skills
  • Commitment to achieving and maintaining Commonwealth directed targets (Key Performance Indicators)

Additional Requirements:

  • Fully vaccinated for Covid-19
  • Meet requirements of Peninsula Health immunisation policy
