Be part of a dynamic team as a Clinical Care Coordinator at a leading healthcare provider.
The role involves coordinating clinical services to support clients in their own home. As the primary point of contact, you will work closely with clients and staff to ensure efficient service delivery and optimal outcomes.
- Daily coordination of care for new and existing clients of a community care program.
- Acceptance of referrals and ensuring all client information is available for service commencement within required timeframes.
- Identifying eligible clients for government funding streams and liaising with referrers for transfer of care.
You will have the opportunity to work with a variety of clients and develop strong problem-solving and time management skills.
Key requirements include:
- Bachelor of Nursing
- Experience in acute nursing
- Community nursing experience
- Full driver's license
- Previous experience managing service contracts and staff
- Strong communication and computer skills
- Ability to work over 7 days per week
