About the Role

The Community Wellness Worker will engage directly with individuals, families, and community groups to identify needs, provide tailored support, and connect people with resources addressing physical, mental, and social health challenges.

Main Responsibilities
  • Work closely with an identified group of individuals and their households to offer personalized assistance in addressing causes of poor health and wellbeing.
  • Employ a holistic approach to address physical, mental, emotional, and social health and wellbeing, fostering resilience.
  • Utilize diverse methods to develop participants' knowledge, skills, and confidence to tackle their own problems and challenges affecting their health and wellbeing.
  • Collaborate alongside people as equal partners, actively listening to their concerns, building trust and rapport, and connecting them with suitable support or interventions.
  • Assist individuals in identifying barriers preventing them from accessing local resources or existing services effectively.
Essential Skills and Qualifications
  • Able to manage a caseload, prioritize tasks, and maintain accurate records.
  • Able to work autonomously within the scope of the role and adhere to legal and ethical boundaries.
  • Able to work collaboratively within teams and liaise with colleagues at all levels.
  • Skilled in daily use of Microsoft Office software and patient/case record systems.
  • Demonstrate strong active listening skills and effective communication with diverse individuals and groups.
