Advanced Practice Nursing Role

As a key member of our multidisciplinary team, the Clinical Nurse Consultant plays a pivotal role in providing clinical expertise and leadership to our Residential Aged Care facility Support Service (RASS) and the Geriatric Emergency Department Intervention (GEDI) in West Moreton Health.
Key Responsibilities:
- Deliver high-quality patient care through advanced practice roles, employing a multidisciplinary approach to screening, triage, assessment, care coordination, and care planning for older people presenting to the Emergency Department (ED) or living within Residential Aged Care Facilities (RACF).
- Educate healthcare professionals on evidence-based best practices for older persons, ensuring safe and effective clinical outcomes.
About You:
- Demonstrate exceptional nursing skills with the ability to engage effectively across various stakeholders, including patients, families, and interdisciplinary teams.
- Showcase contemporary clinical nursing expertise with a deep understanding of patient-centered care principles.
- Demonstrate strong knowledge of relevant legislation and policy, as well as the ability to develop documents supporting their application.
- Develop effective communication skills to build and maintain collaborative relationships across disciplines.
Mandatory Requirements:
- Hold current vaccination status against vaccine-preventable diseases as per the Health Employment Directive NO. 01/16.
