We are seeking an experienced professional to join our team as a Dementia Care Manager. This is a rare opportunity to specialise in dementia care and work with a niche client group.

The successful candidate will be responsible for developing and implementing quality programs and initiatives that support our clients and their families. They will also inspire, guide, and support a multi-disciplinary team to deliver exceptional care and services.

This role requires the ability to manage client referrals, support planning, and ongoing assessment/reviews to ensure appropriate service provision. The successful candidate must also ensure compliance with all contractual requirements and develop services in line with governing quality standards.
