The Child and Youth Mental Health Service in Tasmania is currently seeking an experienced and qualified individual to fill this key role. As a Regional Complex Care Coordinator, you will play a vital part in providing high-quality care to young people and their families, promoting positive outcomes and improving overall well-being.
- Key Responsibilities:
- Providing specialist clinical coordination services to support right place, right care principles.
- Undertaking high-level assessments and developing individual plans that include bringing together the right mix of services across government and non-government agencies.
- Requirements:
- Registered with the Nursing and Midwifery Board of Australia as a Registered Nurse and possess specialist tertiary graduate or postgraduate mental health/psychiatric nursing qualification.
