Key Responsibilities:

  • Deliver expert guidance and support to participants, facilitating discussions and the implementation of NDIS plan objectives.
  • Collaborate with families and participants to enhance their ability to independently manage their supports, which may involve:
    • Identifying suitable services and supports.
    • Providing mentorship and assistance to resolve service delivery challenges.
    • Ensuring that budgets align with service utilization.
  • Monitor participant outcomes, coordinate services, and adhere to the financial parameters of each plan.
  • Build and maintain strong local networks and knowledge of service providers.
  • Perform additional duties as required.
Essential Attributes:
  • Comprehensive knowledge of disability services, NDIS, and inclusion principles, or the ability to quickly gain this knowledge.
  • Strong skills in identifying barriers to support and effectively navigating complex family dynamics to achieve participant goals.
  • Deep understanding of the impacts of disability on individuals and their families.
  • Excellent communication and interpersonal skills, with the ability to manage conflicts and mediate when necessary.
  • Capacity to handle a busy caseload, meet deadlines, and prioritize tasks effectively.
  • Ability to represent the organization positively and cultivate relationships within the community to maximize resources and improve outcomes.
  • Proficiency in managing your own workload autonomously while contributing to the Support Coordination team.
  • Strong technological skills, including experience with client information management systems and Microsoft Office.
  • Ability to work both independently and collaboratively with team members and external stakeholders to ensure seamless service delivery.
Additional Requirements:
  • Relevant qualifications and/or equivalent industry experience.
  • Possession of a Blue Card, NDIS Worker Screening Card, and suitable Police Certificate, or the ability to obtain these.
  • Current Australian Driver's License.
  • Commitment to the organization's values and philosophy.
