Peer Carer Facilitator

Cornerstone Medical Recruitment is seeking a passionate and experienced Carer Support Program Facilitator to join our team. As a Carer Support Program Facilitator, you will play a vital role in supporting Carers in a group setting, drawing on peer support principles and the CHIME framework.
Key Responsibilities:- Facilitate group sessions to achieve program KPIs.
- Implement targeted programs for Young Carers and adult Carers.
- Create a welcoming and inclusive environment for Carers.
- Build positive relationships with Carers and team members.
- Work autonomously while being a flexible and supportive team member.
- Personal lived experience as a young Carer or adult Carer is mandatory for this role.
- Certificate IV or above in a support discipline and/or equivalent community sector experience.
- Experience working with young people/young Carers is a plus.
- Current valid Driver's License.
- Satisfactory National Police Records Check.
- QLD Blue Card.
- QLD Yellow Card/NDIS Workers Screening Check.
- Right to work within Australia.
- NDIS Worker Orientation Module Certificate.
