Carer Support Program Facilitator

Cornerstone Medical Recruitment is seeking a passionate and experienced Carer Support Program Facilitator to join our team. As a Carer Support Program Facilitator, you will play a vital role in supporting Carers in a group setting, drawing on peer support principles and the CHIME framework.

Key Responsibilities:
  • Facilitate group sessions to achieve program KPIs.
  • Implement targeted programs for Young Carers and adult Carers.
  • Create a welcoming and inclusive environment for Carers.
  • Build positive relationships with Carers and team members.
  • Work autonomously while being a flexible and supportive team member.
Requirements:
  • Personal lived experience as a young Carer or adult Carer is mandatory for this role.
  • Certificate IV or above in a support discipline and/or equivalent community sector experience.
  • Experience working with young people/young Carers is a plus.
  • Current valid Driver's License.
  • Satisfactory National Police Records Check.
  • QLD Blue Card.
  • QLD Yellow Card/NDIS Workers Screening Check.
  • Right to work within Australia.
  • NDIS Worker Orientation Module Certificate.
