Job Description

The Social Inclusion Support Worker is a key role within Cumberland City Council's Seniors and Disability Services. The successful applicant will provide care and support to older people or individuals with disabilities, assisting them with daily tasks and improving their quality of life.

Responsibilities
  • Provide personalized care and support to clients in their homes, at Council venues, or in the community.
  • Develop and implement individualized care plans to meet the unique needs of each client.
  • Collaborate with multidisciplinary teams to ensure comprehensive care and support.
  • Maintain accurate records and documentation, adhering to Council's policies and procedures.

Key Qualifications:

  • Certificate III in Aged Care or equivalent experience in a relevant field.
  • Excellent communication and interpersonal skills, with the ability to build rapport with diverse individuals.
  • Demonstrated organizational and time management skills, with the capacity to manage competing priorities.
  • Ability to work effectively in a team environment, promoting collaboration and mutual respect.
