We are seeking a skilled and dedicated Aboriginal Health Worker to join our team at Barossa Hills Fleurieu Local Health Network. As a key member of our team, you will provide holistic support services to Aboriginal clients in the Gawler community, working collaboratively to increase continuity of care and address social issues that impact health outcomes.
Key Responsibilities- Provide culturally sensitive and responsive support services to Aboriginal clients, including facilitation of Well Health Checks and networking with service providers and community organizations.
- Contribute to service development activities, evaluating service effectiveness and monitoring care plans to ensure relevant and appropriate care outcomes.
- Collaborate with multidisciplinary teams to promote positive health outcomes and address the social determinants of Aboriginal health.
- Develop and maintain relationships with Aboriginal community members, service providers, and non-government agencies to enhance service delivery and community engagement.
- Essential Aboriginal and/or Torres Strait Islander heritage.
- Relevant qualifications and experience in Aboriginal health, community development, or a related field.
- Strong communication and interpersonal skills, with the ability to work effectively with diverse stakeholders.
- Ability to work in a fast-paced environment, prioritizing tasks and managing multiple responsibilities.
