Senior Care Coordinator

We are seeking a passionate and skilled Wellbeing Manager to coordinate holistic care delivery across the Queensland region on a full-time, 12-month contract.
A Day in the Life:
- Coordinate tailored Home Care Package (HCP) plans and manage care delivery, ensuring person-centred care at home.
- Provide nursing care one day per week, including assessments, treatments, and care planning as part of the Wellbeing Manager role.
About You:
- MUST HAVE Current Registered Nurse (AHPRA) registration with hands-on experience supporting older adults to live independently.
- Strong case management skills, ideally in community/aged care, and a goal-oriented approach.
- Excellent communication and stakeholder engagement skills, valid driver's licence, and willingness to travel locally.
Key Responsibilities:
Care delivery coordination, nursing care provision, assessment, treatment, and care planning.
