Brief Summary:

This role provides personal care to participants in a center and community, exercising independent judgment. It involves supervising direct reports, coordinating schedules, and carrying out infection control precautions.

Responsibilities:

  • Assist participants with daily needs at the center and community under supervision.
  • Contribute to care plan development through interactions with the Interdisciplinary Team.
  • Collaborate with the team to ensure appropriateness and continuity of care.
  • Carry out non-skilled treatments, including transfers, toileting, bathing, dressing, and assist with individual and group programs.
  • Maintain a clean and safe working environment.
  • Oversee direct reports' tasks and assignments, regularly meeting with staff.
  • Participate in daily cleaning and disinfecting of the center according to guidelines.
  • Provide feedback to direct reports on their performance and keep supervisors informed of any issues.
  • Attend IDT meetings, tracking initial services and changes to service needs.

  • Essential competencies include health aide and office coordination skills.

Qualifications:

  • High school graduate or equivalent.
  • Certification as a Home Health Aide or Nursing Assistant desirable.
  • CPR and First Aid Certification required within 60 days of employment.
  • 2-4 years of experience working with a frail or elderly population, including one year in a lead or supervisory capacity.
