Client Care Coordinator

We are seeking a skilled and compassionate Client Care Coordinator to join our team. The ideal candidate will possess excellent communication skills, be highly organized, and have experience working in the disability care sector.
About the RoleThis is a key position within our organization that requires someone who can provide high-quality care management advice and assistance to senior staff members. The successful candidate will oversee care management for clients with varying disabilities and/or trauma, ensuring their needs are met and they receive individualized support.
- Coordinate and implement care plans for Support Workers
- Provide regular supervision to Support Workers on home visits
- Ensure timely incident reporting and compliance with regulatory frameworks
- Liaise with other health professionals to ensure clients receive comprehensive care
- Maintain accurate records and documentation of client care and support
The successful candidate will possess:
- Australian citizenship or permanent residency
- Bachelor's degree in Human Services, Social Work, Special Education, Psychology, or similar field
- Demonstrated knowledge of legislation, policy, and practice in the disability care sector
Possession of or willingness to obtain:
- Driver's license
- Covid-19 vaccination certificate
- First Aid & CPR certification
- Working With Children Check (QLD Blue Card) & QLD NDIS Worker Screening Check
- LCS2 Background and History Check
