About the Role

We are seeking a dedicated and compassionate Care Coordinator to join our team at Right at Home Southern NSW. As a Care Coordinator, you will play a vital role in ensuring our clients receive high-quality care and support in the comfort of their own homes.

Key Responsibilities

  • Build strong relationships with clients and their families to understand their unique needs and preferences.
  • Conduct thorough assessments of client needs and resources to identify areas for support and development.
  • Develop and implement personalized care plans that meet the unique needs of each client.
  • Maintain accurate and up-to-date records of client interactions and care plans, ensuring compliance with relevant standards and regulations.
  • Collaborate with our team of Home Care Workers to ensure seamless delivery of care and support services.
  • Provide ongoing support and feedback to Home Care Workers to ensure they have the resources and guidance needed to provide exceptional care.
  • Stay up-to-date with industry developments and best practices to ensure our clients receive the highest level of care and support.

Requirements

  • 3+ years of experience in a care coordination or support services role.
  • Sound understanding of the Aged Care Quality Standards and NDIS Practice Standards, Quality and Safeguarding Framework.
  • Excellent communication and interpersonal skills, with the ability to build strong relationships with clients, families, and colleagues.
  • Organizational and administrative skills, with the ability to manage multiple priorities and deadlines.
  • Ability to work independently and as part of a team, with a strong focus on delivering exceptional client outcomes.
