About the RoleThe Youth Outreach Housing Specialist will work closely with young people to empower them, build their agency, and address their identified housing and wellbeing needs. This role involves extensive outreach work, engaging with young people in crisis accommodation, street outreach, and refuges, and translating initial engagement into a strong ongoing partnership.
Key Responsibilities- Meet individual case performance targets through our weekly, monthly, and quarterly dashboard system.
- Provide high-quality, client-centred practice in all interactions, consistent with our values and vision.
- Deliver exceptional case management practices and record-keeping that meet operational requirements.
- Employ a positive and assertive outreach approach to engage clients and facilitate a client-led assessment and Care Plan.
- Provide targeted interventions and family-facilitated group meetings to achieve client goals.

- Current Bachelor's Degree in Social Work, Human Services, Social Science, Health, Allied Health, Youth Work, Community Services, Family Intervention, or Criminology with 4 years' relevant experience in family support, child protection, youth support, or a related field.
- Or, a current Diploma in Community Services, Social Work, Human Services, Child and Family, Allied Health, Youth Work, Child and Family Intervention with 5 years' relevant experience in family support, child protection, youth support, or a related field.
- At least 3 years of experience in a case management role, with a demonstrated ability to work in a performance-driven environment and build client agency and self-efficacy.
- Requirement to travel across the Logan, Bayside, and Brisbane region, as per program/resident location.
- Current drivers' licence and valid police check.
- Flexibility to work outside core business hours in meeting individual client needs and providing responsive care.
- Working with Children's Card (Blue Card) or eligibility to apply.
