Senior Care Professional

**Job Summary:**
We are seeking a highly skilled and compassionate Senior Care Professional to join our team in the Riverland region. As a key member of our care delivery team, you will be responsible for providing high-quality, person-centred care to older Australians, enabling them to remain independent and healthy in their own homes.
**About the Role:**
This is an exciting opportunity to make a positive impact on the lives of our consumers. As a Senior Care Professional, you will work closely with consumers, their families, advocates, and carers to identify their care needs and goals, and develop tailored service plans that empower them to live independently at home.
You will design flexible, strengths-based care solutions, ensuring each plan aligns with budget and risk parameters while focusing on the consumer's unique requirements. Your expertise will be invaluable in delivering innovative consumer-directed care that promotes independence, dignity, and well-being.
**Responsibilities:**
* Provide high-quality, person-centred care to older Australians in their own homes* Work collaboratively with consumers, their families, advocates, and carers to identify care needs and goals* Develop and implement tailored service plans that promote independence, dignity, and well-being* Design flexible, strengths-based care solutions that meet individual needs and circumstances* Ensure all care delivered aligns with budget and risk parameters*
**Requirements:**
* Must be enrolled or eligible for enrolment as a Nurse with the Nursing and Midwifery Board of Australia (for ENC/END classification)* Hold or be eligible to hold a current practising certificate (for ENC/END classification)* Previous experience in providing nursing care in a healthcare setting (for ENC/END classification)* Qualification in health, aged care, or primary health care promotion at TAFE SA Certificate 3 level equivalent or above (for OPS2 classification)* Experience in the delivery of innovative consumer-directed care for individuals (for OPS2 classification)* Significant experience working in a community-based setting (for OPS2 classification)* Experience working within a multidisciplinary team setting (for OPS2 classification)* Experience working with people with complex needs (for OPS2 classification)* Experience using personal computers and computerised client information systems*
