Service Coordinator Role
We are seeking a highly skilled Service Coordinator to join our team. As a Service Coordinator, you will be responsible for providing information and communication with clients and representatives regarding available services.
Your responsibilities may include but are not limited to:
- Providing information and communication with clients/representatives regarding available services.
- Scheduling new service requests as per support worker and client availability.
- Attending to incoming calls from support workers and clients with scheduling and service-related enquiries.
- Advising clients and carers in a timely manner of changes to care and service delivery.
- Liaising with the team in relation to rostering/payroll discrepancies
- Maintaining filing of documentation and general administrative tasks.
- On boarding of new clients and new staff.
- Manages ongoing and one off bookings within the rosters and replaces staff as required.
- Rostering with client requirements and preferences prioritized wherever possible.
- Making adjustments to the roster on a daily basis, in accordance with any changes that have occurred over night or on the weekend.
- Updating employee availability on a regular basis.
- Processing and entering leave forms that have been approved into the system.
- Working closely with colleagues, clients and external service providers.
- Establishing and maintaining positive relationships with staff and other stakeholders.
Requirements
- Certificate IV in Aged Care or related field (or similar qualification).
- Previous experience working in case management, preferably with NDIS or aged care clients.
