This is an exciting opportunity to support individuals with psychosocial disabilities and mental health issues. You will participate in and engage in social and community activities that promote independence and well-being.
We are seeking highly motivated and passionate individuals who share our commitment to delivering high-quality, person-centered services. As a Mental Health Support Worker, you will play a vital role in assisting participants to achieve their NDIS goals and maintain their engagement in the community.
Our participants have diverse support needs, and your role may involve assistance with accessing the community, group activities, companionship, maintaining relationships, and transportation. You will work collaboratively with our team to deliver tailored support that meets the unique needs of each participant.
The successful candidate will be available for a minimum of 3 days per week and possess a strong commitment to providing exceptional support. A valid Australian driver's license, comprehensive insurance, and Working with Children's Check are essential requirements. We also require First Aid and CPR certification and COVID-19 training. - Provide support to participants in line with Primacy Care's social model of recovery.
- Empower participants to make choices in all their lifestyle activities and meet daily living needs.
- Enable participants and build their capacity.
- Provide short-term focused interventions, psychoeducation, support, and guidance to participants.
- Support and assist participants to achieve their goals as per their NDIS funded supports.
To be successful in this role, you will require excellent communication skills, the ability to work autonomously, and a strong commitment to providing exceptional support.
