Lead Patient Care Specialist

Job Summary:
We are seeking a highly skilled and dedicated Enrolled Nurse to provide exceptional patient care.
Key Responsibilities:
- Delivering high-quality patient care and implementing individualized care plans
- Monitoring patients' health, administering medications, and performing necessary treatments
- Supervising and supporting nursing staff in providing optimal patient care
- Collaborating with healthcare professionals, families, and multidisciplinary teams to ensure person-centered care
- Assessing and managing patients' chronic conditions and ensuring timely medical interventions
- Maintaining accurate clinical documentation and ensuring compliance with aged care regulations
- Educating patients, families, and caregivers on health management and well-being
- Ensuring infection control protocols, workplace safety, and best clinical practices are adhered to
Requirements:
- Bachelor's Degree or Diploma in Nursing (or equivalent qualification)
- Current AHPRA registration as a Registered Nurse
- Experience in palliative care, dementia care, and rehabilitation nursing
- First Aid and CPR certification
- Understanding of My Aged Care and NDIS frameworks
- Minimum 2 years of work experience in aged care, geriatrics, or clinical nursing
