Delivering Patient-Centred Care

We are seeking a compassionate and skilled professional to join our general medicine team as a Social Worker. This is an exciting opportunity for someone who is passionate about making a meaningful difference in a healthcare setting.

This full-time, fixed-term position will be working with a supportive and collaborative multidisciplinary team to deliver high-quality clinical social work services to patients. You will play a key role in supporting patients and their families through complex health journeys.

To succeed in this role, you will need to have demonstrated skills in social work assessment, case management, and discharge planning. A strong commitment to delivering patient-centred care in a healthcare setting is essential. You will also need to have well-developed communication skills both written and verbal, alongside the interpersonal capability to engage effectively with patients, families, and colleagues.

The ideal candidate will be eligible for membership with the Australian Association of Social Workers (AASW) and hold a relevant tertiary qualification in Social Work. They will also have experience working collaboratively as part of a multidisciplinary team and have a strong professionalism, empathy, and dedication to achieving positive outcomes for patients.
