Support Coordinator

Job Summary

We are seeking a Support Coordinator to assist individuals with permanent and significant disability in achieving their goals. The successful candidate will work closely with participants to develop and implement plans that promote choice and control.

Key Responsibilities
  • Assist individuals with disability to apply for NDIS supports and services
  • Collaborate with participants to develop goals and connect them with local community resources
  • Support participants to navigate and optimise their engagement with the NDIS
  • Improve community knowledge of the NDIS and create new opportunities to connect participants to services and support
Requirements
  • A positive and respectful person who can build and nurture relationships easily and confidently
  • An approachable and empathetic individual with strong personal values based on compassion and understanding of diverse needs within a local community
  • Demonstrated relevant experience and/or lived experience
  • Experience in planning with people to achieve collaborative outcomes
  • Knowledge of the local community and available options for social and economic participation for individuals with disability
  • Demonstrated computer skills including capability with Microsoft Office 365
  • Ability to hold or currently hold a Working with Children and/or a Vulnerable People Check
