About the Role

We are seeking a values-driven and proactive Specialist Family Violence Outreach Practitioner to join our team. As a key member of our Family Violence Stream, you will provide community outreach case management support to women, families, and children who have experienced and/or are currently experiencing family violence.

Key Responsibilities
  • Work under the general direction of the Team Leaders and the Program Manager, adhering to established work practices while exercising initiative and judgment.
  • Maintain effective relationships with clients that encourage self-determination and participation.
  • Deliver highly effective and appropriate case management to address individual needs and circumstances.
  • Conduct ongoing risk assessment and safety planning for clients using agreed tools.
  • Develop individual case plans in collaboration with clients.
  • Regularly monitor and review progress and outcomes of individual case plans.
  • Assist clients in accessing support services and establishing community supports.
Requirements
  • A tertiary qualification in Social Work or a related field.
  • Experience working in the family violence sector is desirable.
  • Demonstrated case management experience and skills.
  • Highly developed time management and work prioritization skills.
  • Advanced computer skills including experience in data and record management.
  • Well-developed liaison and communication skills.
  • A C Class Drivers Licence.
  • Possess current Apply First Aid qualification (Preferred but not essential).
