
Palliative Care Clinical Coordinator

The position of Palliative Care Nurse Navigator plays a crucial role in delivering comprehensive support to patients with complex health conditions.
This integrated approach ensures patients receive the most appropriate care when and where they need it, working across system boundaries to actively advocate and improve health literacy.
Key Responsibilities- Coordinate patient care across all specialty services, focusing on those with multiple comorbidities and high-frequency presenters for unscheduled care.
- Facilitate a safe and efficient patient journey through the reduction of service fragmentation, educating and empowering individuals.
This role requires advanced clinical and professional practice capabilities to deliver a positive patient-centred health experience.
Candidates must demonstrate capabilities that support effective coordination of patients with complex care needs.
