We are seeking a dedicated professional to join our team as a Palliative Care Nurse Navigator. This role requires an experienced and compassionate nurse who is looking to step into a leadership position that truly changes lives.

This is your opportunity to deliver person-centred care, support patients and families with complex needs, and work across sectors to coordinate holistic, high-quality care during some of life's most vulnerable moments. Our ideal candidate will have the skills and qualifications to provide expert-level healthcare co-ordination, improve health literacy, and advocate for patients and families.

Your Opportunity
  • The Palliative Care Nurse Navigator Service is a state-wide model of care designed to provide comprehensive support to patients with complex health conditions. This integrated approach to healthcare service delivery ensures that patients receive the most appropriate care when and where they need it.
  • The successful candidate will employ advanced clinical and professional practice capabilities to deliver a positive patient-centred health experience. They will facilitate a safe and efficient patient journey across the care continuum by reducing service fragmentation, educating and empowering individuals, and coordinating patient care.

Our Offer

You will receive a competitive remuneration package comprising salary rates $66.86 - $72.59 p.h., employer contribution to superannuation (up to 12.75%), and annual leave loading (17.5%).
