Pastoral Care Coordinator

This role provides spiritual care and leadership to clients, their families, and team members. The ideal candidate will have a strong understanding of people living at home with life-limiting illness, including dementia, and the impact on carers.

The successful candidate will be able to pastorally care for and enhance the wellbeing of those in need, while working collaboratively with team members to extend their skills in meeting the spiritual needs of clients.

Key Responsibilities:
  • Assess client/family's spiritual and emotional needs and develop a spiritual care plan.
  • Provide spiritual and emotional support to clients and their families.
  • Liaise with relevant religious/spiritual leaders to coordinate provision of spiritual support to clients and their families.


Requirements:
  • A commitment to Hammond Care's Christian Mission and Motivation.
  • A strong personal and active Christian faith and church involvement.
  • Theological/Pastoral Care qualifications and experience.
