Support Coordinator Role
At our organisation, we have a unique opportunity for an experienced Support Coordinator to join our team. This critical role enables direct engagement with customers in a professional and meaningful capacity, providing access to the right support and promoting full participation in the community.
Key Responsibilities
- Assist with advocacy on behalf of customers and their families/carers.
- Coordinate a range of supports for customers within the office, community, or in their homes.
- Implement and participate in NDIS planning and review of individualised plans to assist customers achieve their goals.
- Develop customer/participant resilience in their network and community.
- Maintain knowledge of organisational programs and team members for support referrals.
Required Skills and Qualifications
- A solid background working with adults/children with disabilities and their families.
- Demonstrated ability to develop strong, professional relationships with customers and their families, holding exceptional communication skills.
- The ability to problem-solve, show initiative, and contribute to continuous improvement.
- Outstanding interpersonal and networking, negotiation, and consultation skills.
- A solid understanding of and experience in working within the NDIS framework.
- The ability to work autonomously and independently, manage a full case load, and meet deadlines.
- A thorough understanding of person-centred and strengths-based approaches.
- Qualifications/experience in disability, social work, education, leisure, and recreation, or related disciplines (desired).
