Job Title:

Clinical Nurse Consultant - Regional Complex Care Coordinator

Job Summary:

The Department of Health Tasmania is seeking a highly skilled and experienced Clinical Nurse Consultant to join our team as a Regional Complex Care Coordinator. This is a fixed-term full-time position working 76 hours per fortnight, commencing as soon as possible for a period of twelve months.

Key Responsibilities:
  • Provide specialist clinical coordination services to support right place, right care principles and facilitate determined need.
  • Undertake high-level assessments and develop individual plans that bring together the right mix of services across government and non-government agencies.
  • Promote a positive working culture by developing effective partnerships and exceptional working relationships with all stakeholders.
  • Work collaboratively with a multidisciplinary team to provide support and consultation as required.
Requirements:
  • Registered with the Nursing and Midwifery Board of Australia as a Registered Nurse.
  • Possess specialist tertiary graduate or postgraduate mental health/psychiatric nursing qualification.
  • Current Tasmanian Working with Children Registration.
  • Current Driver's Licence.
Desirable Criteria:
  • A minimum of 3 years current postgraduate experience in a field relevant to the position.
  • Holds, or is working towards, postgraduate qualifications relevant to the position.
