Care Coordinator Nurse

Job Overview
We are seeking a highly skilled Registered Nurse to join our team in providing holistic patient care. This role requires exceptional coordination skills, incorporating the nursing profession's values of knowledge, theory and practice.
The ideal candidate will have experience in family practice, surgical, palliative care or oncology, with a strong ability to work efficiently as a member of the team and possess leadership and coaching skills.
Key Responsibilities:
- Assess, plan, implement and evaluate the needs of patients and significant others in the delivery of care.
- Collaborate with all members of the interdisciplinary team in providing patient care.
- Work effectively in a dynamic environment, maintaining a good attendance record.
- Maintain confidentiality and demonstrate a high regard for it.
Required Skills and Qualifications:
- Diploma in Nursing or a Bachelor Degree in Nursing;
- Registered (active) with the Nurses Association of New Brunswick;
- A minimum of two years in the last five years of specialized nursing experience in the areas of family practice, surgical, palliative care or oncology.
- Knowledge of the target population of Extra-Mural Services and available community services.
- Valid New Brunswick Class 5 Driver's License and good driving record.
- Clean criminal record check.
- CPR Level C certification.
- Excellent interpersonal skills, good judgment and decision making skills.
