As a RN Case Manager, you will be responsible for planning, coordinating and delivering quality hospice care while ensuring compliance with federal, state, and local regulations.
Key Responsibilities:- Provides ongoing assessment and nursing care to patients in homes and healthcare facilities.
- Ensures accurate evaluation of patient needs, develops and implements a comprehensive care plan that addresses current needs.
- Follows current nursing professional standards of practice and protocols for hospice.
- Delivers quality care and symptom management for the comfort of patients.
- Communicates with other healthcare providers about changes in condition and patient needs.
- Assesses patient and caregiver needs and provides education when necessary.
- Emphasizes high-quality customer service for internal and external customers.
- Oversees the implementation of the patient's Plan of Care and evaluates its effectiveness on an ongoing basis.
- Current unencumbered license to practice as a Registered Nurse (RN) required.
- One year experience in acute care or medical-surgical nursing required.
- Experience in hospice is preferred.
- Proficiency with technology, including Microsoft Office and electronic medical records (EMR), is required.
- Sitting - Up to 4 hours/day
- Standing - Up to 8 hours/day
- Walking - Up to 6 hours/day
- Lifting, transferring, pushing or pulling patients or equipment/supplies - Up to 50 pounds
- Driving - Up to 6 hours/day
- Work weekends, evenings, and holidays - Required as scheduled
- On-call availability - 24/7 for emergencies
