On-Call Community Care Liaison

Job Description:

We are seeking a dedicated and experienced Part-Time On-Call Community Care Liaison to join our team. As a key member of our community care sector, you will be responsible for managing schedules, handling incidents, and providing compassionate care to our clients.

Your responsibilities will include responding to all incoming requests outside of business hours, including incident management and response, handling staff call-ins, and overseeing scheduling needs. You will ensure incidents are documented and communicated promptly, keeping both participants and team members informed of any changes.

This role requires accurate and consistent reporting of call logs, with the requirement of 1-2 days per week in-office. Your on-call duties will start at 5 pm and end at 9 am the following day, with weekend on-call duties requiring availability 24/7 for the duration of assigned shifts.

You will work on a rotating roster of 5 days on and 3 days off, maintaining flexible availability Monday through Sunday.

Required Skills and Qualifications:
  • Certificate III qualification (Diploma preferred)
  • Previous experience in the community care sector is essential
  • Confident in managing rosters and handling incidents effectively
  • Proficient in Microsoft Office and Adobe products
  • Excellent communication skills, both written and verbal
