**Transition Career Coach**

 

We are seeking an experienced Transition Career Coach to join our team. In this role, you will work closely with students and relevant stakeholders to support young people in their transition from youth detention facilities to employment, training and/or educational pathways in the community.

The successful applicant will actively promote pathways for students to continue with further learning, training and employment opportunities after release from detention. This will involve collaborating with students, carers, government agencies and other service providers to ensure a smooth transition into education and employment.

To achieve this goal, you will need strong communication and interpersonal skills, as well as the ability to collect and analyze data, review programs and evaluate outcomes. You will also be required to work closely with the Head of Department to enact the school's strategic plan and implement innovative practices.
