About This Role

We are seeking a skilled Clinical Health Advocate to join our team and make a meaningful impact in the lives of older Australians.

As a Clinical Health Advocate, you will play a vital role in guiding clients through an 8-week journey in our Short-Term Restorative Care program. Your primary focus will be on restoration and empowerment, helping clients reclaim their independence and confidence.

  • You will visit clients at home to co-design personalised care plans that focus on reablement and wellness.
  • You will run engaging group sessions that promote healthy living and self-management.
  • You will collaborate with healthcare professionals to deliver holistic care.
  • You will use digital tools for virtual coaching and support.
  • You will build local networks and promote the STRC program in your community.
Your Key Responsibilities

Key responsibilities of this role include:

  • Visiting clients at home to develop tailored care plans.
  • Leading educational sessions on healthy living and self-management.
  • Collaborating with healthcare professionals to deliver comprehensive care.
  • Utilizing digital platforms for remote coaching and support.
  • Broadening community networks to promote the STRC program.
Required Skills and Qualifications

To succeed in this role, you will need:

  • A degree in Nursing or Allied Health, with current registration.
  • At least 3 years' experience in clinical care, health coaching, or care coordination.
  • A passion for health promotion, preventative care, and client empowerment.
  • High-level computer skills for digital platform navigation and documentation.
  • A valid driver's licence and a love for community work.
