Key Role: Respiratory Social Worker

  • Support patients and families with counseling, advocacy and care while driving change through teamwork, research and education.

We are seeking a skilled Respiratory Social Worker to join our Cancer and Chronic Care Team. As a key member of our team, you will provide social work services to patients and families affected by chronic and life-limiting illnesses.

Key Responsibilities:

  • Deliver individualised counseling and case management to patients and families with illness adjustment, end-of-life care and complex discharge issues.
  • Collaborate with multidisciplinary teams to ensure comprehensive patient care.
  • Develop and implement evidence-based programs to enhance patient outcomes.
  • Contribute to the development of policies and procedures related to social work practice.
  • Participate in quality improvement initiatives and contribute to the continuous enhancement of our services.

Requirements:

  • Qualification: Degree in Social Work with eligibility for membership of the Australian Association of Social Workers (AASW).
  • Demonstrated understanding of psychosocial issues associated with acute and chronic illness.
  • Demonstrated capacity to provide counseling and case management with patients/families with illness adjustment, end-of-life care and complex discharge issues.
  • Ability to work autonomously and as part of a multidisciplinary team.
  • Excellent written and verbal communication skills, including report writing and high-level computer literacy.
